These Terms of Service govern your use of Revival Family Chiropractic's services and website. By scheduling an appointment or using our services, you agree to these terms.

By scheduling an appointment, receiving chiropractic care, or using our website at revivalchirocharlotte.com, you agree to be bound by these Terms of Service. If you do not agree, please do not use our services.
These terms apply to all patients and visitors of Revival Family Chiropractic, DBA Revival Chiropractic Charlotte, owned and operated by Dr. Bryan Pries, D.C., located at 5527 Monroe Road, Charlotte, NC 28212.
1.2 Description of Services
Revival Family Chiropractic provides chiropractic care services including but not limited to spinal adjustments, neurological assessments (INSiGHT scans), pediatric chiropractic care, family wellness care, TMJ treatment, and post-accident rehabilitation.
Our services are provided by Dr. Bryan Pries, D.C., a licensed chiropractor in the state of North Carolina. Chiropractic care is not a substitute for emergency medical care. In a medical emergency, please call 911.
1.3 Patient Responsibilities
As a patient, you agree to:
- Provide accurate and complete health history information
- Inform us of any changes to your health status, medications, or insurance
- Follow the care plan recommended by Dr. Bryan Pries
- Arrive on time for scheduled appointments
- Notify us at least 24 hours in advance if you need to cancel or reschedule
- Pay for services at the time of your visit
1.4 Appointment Policy
We want to make care as accessible as possible. Same-day appointments are available when scheduling permits.
Scheduling
Appointments can be scheduled by calling (704) 568-2447, texting (980) 324-3363, or using our online booking form at revivalchirocharlotte.com/book.
Cancellations
We ask for at least 24 hours' notice if you need to cancel or reschedule. This allows us to offer your time slot to another patient who needs care.
No-Shows
Patients who miss appointments without notice may be charged a no-show fee. Repeated no-shows may result in discharge from the practice.
Late Arrivals
If you arrive more than 15 minutes late, we may need to reschedule your appointment to ensure all patients receive adequate care time.
1.5 Payment Terms
Payment is due at the time of service. We accept the following forms of payment:
- Cash, check, and all major credit/debit cards
- HSA (Health Savings Account) and FSA (Flexible Spending Account) cards
- Most major insurance plans (see our Insurance page for details)
Payment plans may be available for patients with financial hardship. Please speak with our front desk team to discuss options. We are committed to making care accessible for every family.
If you have insurance, you are responsible for any copays, deductibles, or non-covered services at the time of your visit. We will do our best to verify your benefits in advance, but final determination of coverage is made by your insurance company.
1.6 SMS Text Message Marketing Terms
By providing your mobile phone number and checking the consent box on our forms, you expressly consent to receive text messages from Revival Family Chiropractic.
Reply STOP to any text message from us, or text STOP to (980) 324-3363. You will receive one final confirmation, then no further marketing texts unless you opt back in by texting START.

No Liability for Delivery
Revival Family Chiropractic is not responsible for delayed or undelivered messages, including those caused by carrier issues, device problems, or network outages.

We reserve the right to modify these SMS terms. Changes will be posted on this page with an updated "Last Modified" date. Continued consent after changes indicates acceptance of the modified terms.
1.7 Email Communications
